DPA model 4061-BM lavalier microphone. This mic was installed on an acoustic guitar, and subsequently removed. It was rarely used. There is a professional solder repair on the cable where a retrofit was made for the guitar installation (see photo). Comes with a two channel phantom power pack. Priced to sell, excellent buy!
